How do I obtain a library signature for my Leave of Absence form?

I need a library signature on my Leave of Absence form! Where do I send it? What are the conditions that must be met to get a signature? When can I expect a response?

Here are the important details you need to know:

  • You may send your Leave of Absence form to library@ciis.edu
  • Please keep in mind that in order for Library staff to sign off on the form you must have returned all checked-out library materials and have zero fines associated with your account. How to log into your Library account
  • If you have returned all library materials and have no fines, you can expect to have the signed document returned to you within 2 days (unless we receive it late Friday night, in which case it may take 3 days)!
  • Please be aware, all library privileges will be suspended for the duration of your Leave, including remote access to electronic resources (books/ebooks, articles, databases, streaming video, etc.).
